Abstract

Identification of Drug Related Problems (DRPs) in treatment is important in order to reduce morbidity, mortality, and drug therapy costs. This will be very helpful in increasing the effectiveness of drug therapy, especially in diseases that are chronic, progressive and require lifelong treatment, one of which is Congestive Heart Failure (CHF). By identifying the cause of DRPs, pharmacists can develop a care plan to overcome DRPs so that they can achieve the expected therapeutic goals. This study aims to determine the type and frequency of DRPs that appear in Congestive Heart Failure (CHF) patients who are hospitalized at Mardi Waluyo Hospital, Blitar City in 2019. This study is a non-experimental descriptive study. Data collection at the Inpatient Installation of Mardi Waluyo Hospital, Blitar City, was carried out using a retrospective method. Identification of Drug Related Problems (DRPs) based on the PCNE V9.0 classification of cause and problem categories. The results obtained from 90 patients there were 338 events of DRPs which included 21 events (6.16%), adverse drug events (possibly) occurred as much as 1 event (0.29%), drugs without indications as many as 5 events (1.47%) , drug interactions were 309 events (90.62%), duplication of drugs with the same active ingredient or drug group was 1 event (0.29%), dose too high was 3 events (0.88%) and duration of therapy was too short as much as 1 event (0.29%). Conclusion: based on the PCNE V9.0 classification, the category of causes and problems of DRPs that most commonly occurred at the Mardi Waluyo Hospital, Blitar City in 2019, was drug interactions.
