Abstract

Hypertension is referred as the silent killer because it shows almost no symptoms and cannot be diagnosed before carrying out blood pressure checks. Hypertension was defined by the value of systolic blood pressure > 130 mmHg and diastolic blood pressure > 80 mmHg. Antihypertensive therapy is aimed at controlling blood pressure by modifying a healthier lifestyle accompanied by the administration of single or combination antihypertensives. The benefits of choosing a good and correct treatment are very influential, especially for patients who have been taking drugs for a long time, or for life such as hypertension patients. . This study aims to analyze the cost-effectiveness of combination antihypertensive therapy in Mardi Waluyo Hospital, Blitar City. This study uses cross-sectional as research design based on a hospital perspective using 30 secondary data of inpatient hypertensive patients with the type of BPJS financing for the 2019 - 2020 period. Data analysis includes demographic data, length of hospitalization and total direct medical costs. A total of 12 patients used a combination of furosemide - bisoprolol, 10 patients used a combination of furosemide - amlodipine, and 8 patients used a combination of furosemide - candesartan as antihypertensive therapy. The age of the patients ranged from 45 years to > 65 years. The effectiveness of combination antihypertensive therapy was measured based on systolic and diastolic blood pressure, Mean Arterial Pressure (MAP), and Length of Stay (LOS). Statistical analysis was performed using the Kruskal Wallis test. The combination therapy for antihypertensive furosemide – bisoprolol has a percentage effectiveness value based on 75% systolic and diastolic blood pressure, 50% MAP and 6.7 days LOS with the lowest ACER value based on systolic and diastolic blood pressure of Rp. 112,345, and based on the Mean Arterial Pressure (MAP) of Rp. 168,518. ICER value based on systolic and diastolic blood pressure is Rp. 100,402, the ICER value based on the Mean Arterial Pressure (MAP) is Rp. 33,467, and the ICER value based on the Length of Stay (LOS) is Rp. 12,550,310. Based on the results of the study, it can be concluded that furosemide-bisoprolol was cost-effective as antihypertensive therapy. Keywords: Cost effectiveness, hypertension, antihypertensive.
